“The Wade Creek Clinic” is Now Accepting New Patients.

Student hours:  Monday, Tuesday, Thursday Friday 8 to 12.
Student walk- ins welcome, but appointments are encouraged

Community Hours: Monday, Tuesday, Thursday, Friday 1:30 to 5

Students should be encouraged to come to the clinic to set up an appointment.  Sometimes same day appointments are available.  If we are unable to provide a same-day appointment, we can often get students in within a week. Please call 503-630-8550 during regular business hours to set up an appointment.

Services
A
t
the “Wade Creek Clinic” we care about our students’ academic careers. An important role we play is to provide health care that minimizes interruptions to a student’s education.
We facilitate access to comprehensive preventive, primary and mental healthcare for students  enrolled in the Estacada School District as well as provide access for other rural students from the surrounding areas such as Colton, Redland, George, and Eagle Creek.

   Routine physical exams, including sports physicals
   Early detection, diagnosis and treatment of illness and injury
   Vision, dental and blood pressure screenings
   Immunizations
   Mental health services
   Age-appropriate reproductive health
   Routine lab tests
   Prescription medications
   Health education, counseling and wellness promotion
   Fitness and nutrition education and counseling
   Referrals for healthcare services not provided at the clinics

Future plans will include limited dental services.

Vaccines
The clinic is currently able to offer certain vaccinations to eligible students.  Students, parents or guardians should contact the clinic for more information.  For minors under the age of 15, we require parent or guardian consent in order to administer vaccinations.

Insurance
The School Based Health Center is for ALL Students regardless of insurance status!!
Estacada School and Community Based Health Center is funded through medical insurance, third-party insurance, Medicaid, grants, private donations, and public funds. Providing us with your insurance information at the time of your appointment allows us to bill your insurance, which reduces the amount of public funding we need to provide service to as many students as possible.

At Wade Creek Clinic students who do not have insurance may be asked to pay a small co-pay towards care, although no student will be turned away for the inability to pay. 

Community Members and Families with no health insurance or those individuals that do not provide insurance information may qualify for a screening to determine if they qualify for the Oregon Health Plan/Healthy Kids Oregon Health Plan  or other insurance program's. These programs could provide coverage for your child for medical, dental, and emergency services. We strongly encourage you to apply for this valuable coverage. Please click on the link below or call.

Affordable Health Insurance Coverage 

Since October 2013, individuals and families who do not have access to affordable health insurance at work are able to use a new marketplace called Cover Oregon to compare and select health insurance plans. 

Through one application process, individuals can enroll in commercial insurance plans or programs such as the Oregon Health Plan or Healthy Kids. Some families may qualify for financial assistance to help them pay for coverage.

   Wade Creek Clinic is part of Estacada School District and operates as a School & Community Based Health
     Care Center with a focus on youth.

Estacada School District’s Wade Creek Clinic has medical outreach opportunities for students in Colton,
     Molalla, Eagle Creek, Boring, George, Redland, and home school students.
   
Wade Creek Clinic is unique in that during non-student hours we serve community members from
     ages 0 to adult.
   
Wade Creek Clinic is a full service primary care medical clinic sponsored by Legacy Health.
   
Wade Creek Clinic staff includes a full time Nurse Practitioner and a Certified Medical
     Assistant/Office Assistant.
